This company billed my Medicare account and my supplemental insurance for $12,150 for supposed medical supplies. I did not order or receive any such equipment. I am contacting both insurance plans about the fraud. I found out because I received a report from my supplemental insurance denying the claim.

Received notice in the mail from MDCR stating “Your claims for Part B(medical insurance). States it is not a bill but may be billed $1556.20 for services (Coude Catheter) on 7/30/25, 8/29/25, 9/30/25, 10/30/25. The name of the Dr supposedly ordering these procedures is on the notice as a Thomas R Cummings. My husband , nor I were in Florida at anytime, nor did he ever have a catheter placed at any time. Claim number used:25315701883000 on notice.

Obtained my Medicare Number with out my knowledge in order to file claims with Medicare for urinary catheters monthly, May 2025 to September 2025 for a total of $11,475.

I never asked for or received any catheters. They never returned my calls when I tried to get in touch with them after I received my Medicare Summary Notice detailing the transactions.

Medicare did not approve any of the transactions.

What I'm concerned about is that they were able to obtain my Medicare number without my knowledge in order to file this fraudulent claim with Medicare..
