I received the attached fax at a fax at my employer claiming an unclaimed life insurance policy from a person with my same last name (see attached) with instructions on how to claim proceeds and the scammer would keep 10% of the money but would assist in recovery. The fax appears to be from a law firm and signed by an attorney.

I am supposed to contact with Edward Mazin within 30 days, to learn how to be in partnership with Edward Mazin, to claim my share of unclaimed permanent life insurance benefits from an individual that died in an accident, and no one has come forward to claim the Sixteen million eight hundred thousand dollars ($16,820,063.00).
