It was addressed to my personal name and address with the mortgage company that I initially used. Have not been associated with that mortgage company in 4 years. Had a Home Owner ID: CH90430970 with a toll-free number to call 888-467-3210. It stated it reserved the right to revoke waiver after 7 days. Please call immediately as this will be our final attempt to notify you. It only gives Monday-Friday 8am to 5pm PST and no Satursdays. The allocated waiver is for $199.00.
