I wanted to download a passport application because I was planning a trip to another county in October 2023. I found the site, GOVERNMENT PASSPORTS, GI, on the Internet, which definitely looked legitimate and appeared to be an official government organization associated with the U.S. Government. After I completed the official DS-11 Form, I was charged $128.95 on my debit card. Then, I downloaded the completed form and I took it to the local Bossier Court House Passport Agency to have my photo taken and to have the DS-11 Application processed. At this time I was first informed that the DS-11 Form was FREE and I should not have paid for the form. I was, also, informed that I had to pay an additional $130.00 to complete the passport process. Incidentally, the passport agent used my completed DS-11 form as it was the official form.

My major concerns are two fold:

1) I had my complete social security number on the application form. I'm fearful that my number will be used fraudulently causing serious problems in the future with my credit.

2) I was mislead by this official-looking govermnent site in which I lost $128.95 for trusting this company. I sincerely hope others do not fall for this scam and check out the website first.

P.S. It is so sad that ANY company or person can copy an official form and use it on an official-looking website to make money for themselves and to con innoncent people.
