I received a letter from Central Office (doesn't say what company) that me that I can receive $5,000.00. I am supposed to return the Process Completion form and $20.00 to cover all costs and referral data. Make checks payable to Imperial Award Services, 6929 JFK Blvd, #20-285, N. Little Rock, AR 72116-5331.
