I received a letter dated 2021-09-16 asking me to sign the attached form so they could bill Medicare. The ambulance transportation on 7/25/2021 was for Alber Rowell, who I do not know.

I have never been transported by ambulance in the Orlando area this year.

I tried to call this establishment and I was unable to leave a message or talk with anyone. I used their local number as well as their 800 number, and was not able to leave a message. I went to their website and sent a message to their customer service to call me, and they have not responded.

Beware of false billing for services not performed.
