I noticed a claim filed on my insurance. I called my insurance company to ask about the claim because I did not receive any medical service on this day. Gazar Recovery Inc filed a claim with my insurance company for "supplies". I never used any supplies or had any dealings with this business. After speaking with my insurance company, the claim was denied, and Gazar Recovery Inc received no money.

SNKL BILLED MY INSURANCE COMPANY, HUMANA, $6,600.00. Humana sent me a letter saying claim was denied. This claim was for sterile fiber gelling dressing, each. I did not go to any doctor on that date. Prior to this I had received from the companies that is associated with insurance companies that there was a breach of their companies. Guess my information is out for scammer to use. I immediately froze my credit reports. I am thankful my insurance denied this claim.

I received an Explanation of Benefits (EOB) that shows there was a fraudulent claim. The company, Gazar Recovery Inc. is claiming to charge Humana for services that were never done. I have never received service from this company and they are claiming to charge $9,500. They are also attempting to charge twice, both at $4,500 for a total of $9,500 in falsified services that I have never received. I failed a grievance and appeal with my insurance company.

I was reviewing my claims and saw a claim from Gazar Recovery, I looked it up and see someone had already reported the exact same thing I am seeing on mine. It was under my son's name, " Had a $6,600 charge show up on my health insurance from Gazar Recovery INC, noticed it by my EOB. Said it was for KO Double Upright Prefab OTS. Looked it up online and it is some type of knee brace. I don't have issues with my knees and i have never heard of or been to Gazar Recovery. Luckily my health insurance company denied the claim and i will be calling them to make sure nothing is paid to Gazar Recovery."

SNKL filed a health insurance claim with Humana for $6,600 for services not provided (sterile fiber gelling dressing). Humana appropriately denied the claim and I confirmed with Humana that the patient is not responsible for the $6,600. I am reporting here so that others are alerted to this insurance scam

SNKL submitted a claim to my insurance company for $6,240. EOB that I received from my insurance company indicated amount allowed under my plan for this service was $1,483.20. Entire amount was to be billed to me. Fortunately, I have not received an invoice for this amount, so no money was paid to SNKL. Contacted my insurance company and indicated it was a fraudulent claim.

I received an EOB from my insurance company for a $6,240 claim for medical equipment for this company. I did not receive nor use any medical equipment. They are attempting to bill me for $2,692 for the allowable amount.

The company named SNKL Services Corp out of Brooklyn, NY created a fraudulent insurance claim for $6,240.00 in my name and my EOB says that they paid $1,719.15 of the claim, leaving me with $4,520.85. I did not use this company nor were they a service provider. They scammed my insurance company out of almost $2,000. Now, I am wondering of they have my personal information, etc. Total scam.
