Unsolicited telemarketer stating that they are calling from Medicare to get vulnerable seniors to provide personal information such as SSN, Medicare Card Number, Date of Birth, Address. Once they get what they want, they send seniors low quality orthotic items. What I find interesting is that an actual physician's name is associated to the bill as "Ordering Physician" and it's always a physician that the patient/Medicare member has never heard of.

Once they send the orthotic items, they send a separate letter with a document called a "Summary of Medicare" that provides the following information

Date of Service.

Name of company providing services.

Ordered By: (Physicians Name) You will have never heard of this particular physician.

The envelope provides the following return address:

Fargo, ND 58108-6715

1. Medicare Fraud.

2. Seniors receive "MSN" aka Medicare Summary Notice, where the column titled "Maximum you may be billed" may show ZERO, but in another few billing cycles will eventually contain an amount a senior will be charged for those items.
