Caller purportedly offering a survey on health care. Many questions recorded with requested answer "Yes" or "No". Final questions included income level, detailed address and name. I declined to respond with scripted choices. Caller requested permission to send name and address to "affiliates". I did not give my name or address. Caller ID number was fake. There is a survey company with a similar name (not the same name) which appears to do web-surveys only.
