I have received several envelopes stating that this is an "Important Document Enclosed" and "Second Notice/Time Sensitive." Each envelope I have had has different P.O. Box numbers on them: P.O.Box 2910 or P.O. Box 100080. Every envelope has a slip with the year on it, and a return envelope. The slip is stating that I am entitled to more benefits not provided by government funds. It also states that I can have access to a 2021 state-regulated program which will pay 100% of all final expe80nses up to $35,000. It states that I need to respond by March 11, 2021. They are asking for personal information like your name, name of spouse, street address (no P.O boxes), and phone number. Mind you they say no P.O. Box, but they sent it to my P.O. Box.
