Overview and working hours:

The company will be sending you the office equipment you will need for this position. This includes a portable desk, a laptop, and a scanner, all of which are essential tools for your role. Additionally, you will undergo two-weeks online training program from the comfort of your home. This training will be conducted via video conference, and you will be assigned a training supervisor who will guide you through the process. As a member of Companion Diagnostics, we would ask for your commitment to deliver outstanding

quality and results that meets and hopefully exceed expectations. In addition, we expect your personal

accountability in all actions and results that you provide as a representative of Companion Diagnostics.

highest level of your ability and potential. We are confident that you will find this new opportunity both

challenging and rewarding. The following points outline the terms and conditions we are proposing.

Job Title: Data Entry Clerk

Benefits & Bonuses: Medical Insurance, Life insurance, Retirement Plans and 401k Plans, Disability

After your training period, You will be required to choose your working hours (Full-Time or Part-Time)

Working hours are absolutely flexible. During your training period, you will receive detailed guidelines

on how to schedule your working hours. You have the flexibility to add an extra 5 hours to your regular

schedule, allowing you to reach a total of 40 hours per week or 25 hours per week with the option to

work overtime.

NOTE: A progression and performance review will be conducted on a quarterly basis to assess

performance to-date, and to clarify or modify this arrangement, as the need may arise. This

arrangement may be terminated by either party upon notice in writing to either party with notice that

complies with Employment Standards (or Labor Standards) for Indiana.

EMPLOYMENT AGREEMENT

This Agreement made as of this day, between Companion Diagnostics [Employer], an institution

incorporated under the laws of the State of Indiana and Sabrina Daves [Employee], hereby put both

parties in mutual relationship, where the employer desires to obtain the benefit of the services of the

Employee, and the Employee desires to render services on the terms and conditions set forth.

• Employment

The Employee agrees that he/she will always faithfully, industriously, and to the best of his/her skill,

• Termination

The Employee may at any time terminate this agreement and the employee’s employment at any time,

for a sufficient cause and by giving two weeks of written notice to the Employer.

• Entire Agreement

This agreement contains the entire agreement between the parties, superseding in all respects all prior

oral or written agreements or understandings pertaining to the employment of the Employee by the

Employer and shall be amended or modified only by written instrument signed by both parties and we

Another person I saw who also got the same job searched on ScamPulse.com and saw they used exact names in the offer letter. Same verbiage for a different company called Interpreta.

Person claiming to be from HR of Companion Diagnostics, Inc. contacted me in regards to a data entry clerk position. They provided salary details, benefits, information about the company, and a link to their website. A day after “applying” they contacted me advising an offer letter was sent, despite not having a formal interview with anyone. When asked why there was no formal interview, I was given no response. The website offers no contact info other than a fill-in form to submit online. There is a LinkedIn profile with only one person attached to it, and it was not the HR assistant that contacted me. Cannot verify mentioned COO. In offer letter received, they advised I had to pay for software access that they would issue a check for, and those funds should be used for the program.
