If you check the address of the company, you will see that it is at a UPS Store in Stamford.

In the very fine print, easily overlooked (almost overlooked), a disclaimer is printed in light, all caps printing: "C. F. S. is not a government agency and does not have a contract with any governmental agency to provide this service."
And then, in heavy black bold print "Business Annual Report(s) may be filed with the West Virginia Secretary of State for the statutory $25 fee."
The form then continues with Step 1 - 6. Step 5 is Payment Information.
Business owners should be aware of this scam and be careful to read all parts of any mailings received pertaining to their legal requirements.
