My wife received a call offering pain relief spray. The caller asked personal identifying information and asked insurance and contact information. When the script arrived, the label lacked pharmacy and physician information. The price charged to my insurance benefit w/ Express Scripts over $500 for generic lidocaine spray w/ multiple refills on file. After some research, the doctor in the information I gathered was listed as Dr. Holli M. Martinez of Warren, Ohio and Frankfort, KY, but the pharmacy used was located in Mt. Juliet, Tennessee. Another woman, Candice Helton of Mt. Juliet, TN was lifted as the owner of the phone number used with the Athena Pharmacy address instead of the information found on their website.
