Sister has received harassing phone calls and mail for a saliva test. Now wants signature to bill ins. Not ordered by Dr. Letter says can only bill ins for consult and signature page states patient must pay lab tests if not paid by ins.phone belonging to address is a chiropractor ofc not affiliated with lab. Person at lab phone very rude.
