Received a single piece of paper with little to no information. The paper has T-2 at left corner which states “2019 Benefit Information For Michigan Citizens Only” below it states “As a resident of Michigan, you are entitled to more benefits not provided by government funds. You now have access to a 2019 state-regulated program which will pay 100% of all final expenses up to $35,000. Return this postage paid card within 5 days to request this new benefit information.” Information they ask to be provided are name, spouses name, age, street address and phone number. In small letters it says “Not affiliated with or endorsed by any government agency.” There is also an opt out mailing website of dmpoptout.com with 9 digit code 696-895-098
