Don’t Call the phone number that listed & ask them to remove your phone number or mailing address, it just makes things even worse. Not sure how they get your information in the first place but I been shredding them..

Looks suspicious no other information ..no reference to selling or premium cost .. or company information
It should be illegal to make a form that looks like a tax form!
